1.0Summary
Every market access landscape ever delivered shares a defect. It stopped being true at some point after the invoice. Fee schedules update. Payers re-issue policies. A Category III code inches toward Category I, or sunsets. CMS pauses an entire coverage pathway, as it did with TCET this spring. The deliverable does not know any of this happened.

The industry's answer is a shrug
The traditional model treats staleness as a feature. When the snapshot ages out, you commission another one. Policy alert services fill some of the gap by telling you something changed, but an alert is only an input. It does not tell you whether the change touches your codes, your payer mix, or your evidence plan, and it certainly does not update the strategy document your board is still quoting.

What a living landscape requires
The fix is architectural, and it has to be built into the original deliverable. Three properties make a landscape re-verifiable.
3.1 Claims with addresses
Every factual statement in the deliverable carries its source and retrieval date. A claim without a source can only be re-researched from scratch.
3.2 A diffable record
The claims live in a structured ledger, so a pipeline can re-fetch each source on a schedule and flag exactly which claims moved, rather than re-reading the world from scratch.
3.3 Interpretation on top of detection
A changed source becomes an alert only after analysis asks whether it alters a coverage position, a rate, or an evidence requirement for this product. Most changes do not. The ones that do deserve a memo.

The test to apply
Whoever builds your next landscape, ask them two questions. First, can you show me every claim in this document with its source and the date you checked it? Second, what does it cost to have you re-verify the whole thing next quarter? If the first answer is no, the second is "the full price again." That is the snapshot economy, and you do not have to fund it anymore.
