1.0Summary
In April 2026, CMS paused the Transitional Coverage for Emerging Technologies (TCET) pathway and directed Breakthrough-designated devices toward RAPID. Months later, we still see strategy decks, investor materials, and consultant recommendations built around securing a TCET slot. For an already-cleared device, that recommendation is now a fossil.

What actually changed
TCET was CMS's mechanism for granting transitional national coverage to Breakthrough devices after clearance. RAPID reorients the timeline toward the pre-market and IDE stage, which moves the coverage conversation earlier in a device's life. The practical consequence is blunt. If your device is already cleared and your access plan said "pursue TCET," you no longer have that plan. If your device is pre-market with a Breakthrough designation, you have an opportunity your competitors may not have noticed yet.

Who should move now
Three profiles should re-read their coverage strategy this quarter.
3.1 Pre-market Breakthrough devices
RAPID's center of gravity sits where you are. Evidence planning done now can be designed against the pathway rather than retrofitted to it.
3.2 Cleared devices with TCET in the deck
The slide needs to come out, and the strategy that replaces it depends on your code status and payer mix, which is a landscape question rather than a policy question.
3.3 Investors holding Breakthrough‑stage paper
Portfolio companies citing TCET in their reimbursement narrative are citing a paused pathway. It is a diligence question worth asking this week.

The uncomfortable part
Pathway changes like this are exactly why static market access deliverables age so badly. A landscape delivered in March that recommended TCET was wrong by May. Any strategy document about an emerging-technology pathway should carry a re-verification plan, because CMS will keep iterating and the companies that notice first will keep winning the timing.
We rebuilt a client's strategy section around RAPID inside a live engagement when the pause landed. That is the standard to hold. A strategy document should be accurate when read, and re-verified until it is retired.
